The Reason We Ask About Smoking at Every Single Check-Up — Not Just Once a Year.

We're not asking to judge. We're asking because the answer changes how we read everything else we see.

If you've been with us a while, you'll have noticed we ask about smoking every single time, even if the answer hasn't changed for years. It isn't box-ticking. Here's what the answer does for us.

It Changes How We Read Your Gums

Bleeding is the main early warning sign of gum disease, and smoking dampens it. Smokers' gums tend to bleed less than non-smokers' gums with the same amount of disease, so a smoker's mouth can look calm while disease is quietly active underneath. When we know, we look harder, measure more carefully, and don't take a healthy-looking mouth at face value.

It Changes the Plan

Healing depends on a good blood supply and oxygen, and smoking interferes with both. Smoking slows healing after extractions and gum treatment, and raises the risk of problems with implants. None of that means "no". It means we plan differently, set honest expectations and time things properly. It matters most before work starts, which is why we'd rather hear about it at the planning stage than discover it at the follow-up.

It's Part of Cancer Screening

Tobacco is one of the biggest risk factors for mouth cancer, and smoking and drinking together raise the risk far more than either alone. We've covered how we check, and what to watch for, in a separate post. Knowing your habits helps us decide how closely to look and how often to see you.

Because Answers Change

People cut down, stop, start again, or switch to vaping. The answer you gave last year may not be today's answer, so we ask every time. We ask about vaping and other nicotine too. They aren't the same as smoking, and the long-term picture is still emerging, but the long-term effect on gums is still being studied, so we still want to know.

What We Do With Your Answer

We don't lecture. If you smoke, we adjust how we monitor you and talk honestly about what it means for any treatment you're considering. If you'd like to cut down or stop, we'll point you to Help Me Quit Wales, the NHS stop smoking service (0800 085 2219, helpmequit.wales). If you'd rather not, that's your call, and we'll still look after your mouth properly.

If You're Thinking About Bigger Work

Implants, gum treatment, veneers and smile makeovers all depend on healing. If you're considering any of them, being open about smoking at the start gives you a realistic plan instead of a nasty surprise later. It's also a big part of why we look after your gums first.
